There is a list of modules used in API section in the forms basic documentation. For this specific example you would need to import following modules:
- CardsModule
- InputsModule
- IconsModule,
- ButtonsModule,
- CheckboxModule,
- WavesModule (optional, for waves effects)
- InputUtilitiesModule(optional, if you want to add input validation)
