How do i create multiple check boxes within a table? I need to be able to choose a "Yes" or "No" answer across multiple rows.

Currently my check boxes only allow me to choose the very first check box and clicking on any of the other check boxes has no affect.

# Activity Yes No 1 I have the correct access level on EPOS 2 I am fully till trained 3 I understand Our Customer Promise (The 3 S's) 4 I understand Manager reports on Food for Thought (FFT) 5 I understand Do the Right Thing! 6 I understand the Dark Knight principles 7 I understand how to use My Stop 8 I have signed the Key Holder Acceptance form 9 I can complete Fire Drills (including conducting the fire meeting point and break glass point tests) 10 I am familiar with the People Manual (including policies on absence, holidays and lone working) 11 I understand My Hermes collections

Maybe try to use radio buttons instead of checkboxes? You can find them here: https://mdbootstrap.com/docs/jquery/forms/radio-button/

With radio buttons, you can choose only one option, so I think it'll meet your expectations.
